POSITION SUMMARY: The Bravo Health Advanced Care Center (BHACC) is seeking a highly organized, friendly Clinical RN to provide clinical support, advice and information to the highest possible standard of evidence based, patient-centered care for patient with a strong emphasis on chronic diseases. The Clinical RN will be required to efficiently multi-task in a fast-paced outpatient care clinic. DUTIES AND RESPONSIBILITIES: Clinical assessment and intervention are a core component of the medical practice and assists in determining the right response at the right time and pace for the patients presenting problem. Comprehensive assessment requires attention to clinical, psychological risk and an intuitive capacity. Appropriate nursing interventions are based on knowledge of best practice, research, and a complex understanding of health system resources and their availability. Provide screening, monitoring and nursing assessment of the patient’s physical and emotional well-being to interdisciplinary teams, and applying recognized guidelines and best practices for same. Responsible for daily patient care, patient/family teaching, discharge of patients to primary care physician, or links patient to primary medical care, assistance with patient transfer issues, coordinates transfer to the Emergency Department for evaluation and treatment of inpatients, available for inpatient procedures, i.e. line placement, etc. with other members of the Hospitalist team. Obtains health histories and performs physical examinations. Initiate and contribute to the development of patient care plan that manages acute and chronic health conditions via evidence based guidelines. Orders, conducts, and assists with diagnostic studies and laboratory tests. Performs diagnostic and therapeutic procedures including but not limited to suturing, wound care, incision and drainage, splinting, casting, biopsies, cryo-therapy, invasive procedures such a line placement, etc. Regularly monitors, evaluates and adjusts the care plan/objectives based on effectiveness of interventions and or changes in condition or environment in collaboration with physician, patient and other team members. Consults appropriate physician(s) for patient care needs & presents & documents patient data. Promotes self-care, health promotion, and disease prevention and assists with billing documentation and billing processes. Develops patient care policies, procedures, or clinical guidelines. Participates in meetings, committees, and projects relevant to the function of the organization. and other duties as directed. QUALIFICATIONS: Completion of a baccalaureate nursing program preferred. Successful experience working in an outpatient clinical setting with challenging high acuity patients for three (3) years. Demonstrated clinical skill in physical examination and nursing intervention. Chronic disease education and management experience. Wound care, disease management, IV hydration, or PICC line experience is desirable. Licensed as a Registered Nurse in the State of Maryland. Current ACLS/BLS certification. Proficiency with Microsoft Office applications (Word, Excel, Outlook, and PowerPoint).
